Undifferentiated Arthritis Revealing Cogan’s Disease

Background: Cogan’s syndrome is a rare autoimmune vasculitis (less than 300 cases described in the literature) characterized its typical form by cochleovestibular involvement, interstitial keratitis, and an inflammatory involvement of large vessels. It frequently accompanied general signs cardio-vascular, rheumatologic, digestive complications. Its management essentially based on use corticosteroids immunosuppressants. The outcome treatment, even if rapid, does not always avoid functional This disease probably under-diagnosed Africa. Objective: objective this study was to report case Cogan sub-Saharan Case report: A 78 year old black African female patient presented with arthritis left knee without fever. examination also revealed bilateral deafness rapidly progressive onset. known be diabetic hypertensive, regularly monitored. biology joint puncture fluid. Pure tone audiometry confirmed 77% hearing loss. ophthalmological stromal oedema hypertensive retinopathy Kendall stage III. rheumatological evolution good under methylprednisolone methotrexate, but persisted. occurrence fever day 12 treatment required further investigations (blood culture, PCR-Covid 19 test, cytobacteriological sputum, thoracic CT). unfavourable death patient. Conclusion: disease. diagnosis are multidisciplinary. can disappointing.
